First Appointment Checklist

We are excited that you have decided to take the first step towards fertility treatment with Mount Sinai Fertility. During your first appointment, you will be introduced you to a few of your immediate fertility team members, review your medical history and gain a strong understanding of your unique situation. First visits typically last approximately 30-60 minutes.

 

To make your first appointment as comfortable and efficient as possible, please complete the following steps before visiting our clinic and bring all necessary paperwork and identification:

Intracytoplasmic Sperm Injection
4 weeks before your first appointment

If you have seen another Gynecologist, Reproductive Endocrinologist, fertility clinic or other relevant specialist please bring a copy of your records or please sign a Release of Information with Mount Sinai Fertility prior to your first visit.

Include records from your urologist (if applicable).

1 week before your first appointment

Sign on to the eIVF Patient Portal with the information provided to you by the clinic

Complete the New Patient Intake Form on the Portal

Complete the Patient Consent for Email Communication on the Portal

With your patient consent forms you will find a consent to participate in research form. The clinicians at MSF will be leading and participating in research studies and may request your participation in these studies. Please ask your clinical team for more information or visit our Research page.

It is helpful to write down questions you may have for your first appointment.

If you have a partner(s), they will need to complete these steps as well.

What to bring your first appointment?
  • Printouts of all your medical records (if applicable) and ensure your forms are completed on the eIVF portal
  • Valid Ontario Health Card or UHIP information for each participant
  • Credit card or debit card in case a requested test requires payment
  • It is helpful to bring notebook to write down anything that you need to remember to do.
  • Please note that Mount Sinai is a teaching hospital and there may be learners at your first appointment.
